Create Form Fields and a Payment Gateway for my Website

I need someone to create two form fields (to be chosen by the user—personal or business) that once they are filled in the information is sent to my email address (on Amazon AWS). For the user, after they fill in the form field they are then taken to payment page that will have the prices indicated ($100 for personal and $500 for business). I want the payment gateway to also incorporate a picture of a $100 bill and $500 bill with the description of the $100 for a personal story posting and $500 for a business story posting.

The flow of form fields and payment gateway will start with the action button on the main page of StoryBrancher (see attached screenshot). Uploading the design to the main page is part of this project (I already have the files so no additional design on this page needed just incorporating it on the main page).

I also want the text of the pages indexed for SEO purposes as well so the text can be found by search engine bots. Regarding the payment gateway, I want to use PayPal.

The ideal person for this project should have experience in the following:

MediaWiki (StoryBrancher is built from MediaWiki)
Amazon AWS (the email system I use is on Amazon AWS as well as the site hosting)

For your entry to be considered, I would like to have submissions showing the design of the form fields as well as the design of the PayPal Payment gateway. I want the design to have a similar style and color scheme as the website already has.
