Design a letter head for me

Hi Freelancers.

I require a letter designed for my new business " Niseko Chiropractic " .

It will be used for correspondence with medical professionals and other 3rd parties so I am after a simple efficient design that does not take up too much room on the page.

The header should have my logo, business name and phone and email contact details.

The footer should have full business address, website, email and also some sort of nice colour.

I have attached a previous letter head from an old business that I was happy with (marked MIH letterhead ) although I did find the design at the top just a little 2 big.

I would like 2 documents produced

1. A PDF that can be feed through a printer and will produce good letters when a blank document template is filled in and printed from word.

2. A custom word letter head template that allows me to write a letter in word and email or print it.

I have attached a draft version ( It sucks ! ) just to give you the details of the website and email address etc.

I would also like you to share the raw IA or indesign files if used as well.
