Website ReDesign & Development - need done cheaply (with forms and database)

Hi, Please submit your bid for the following along with samples of your work if you can provide: Our Health Source website [login to view URL] needs to be re-designed and fuctionality needs to be edited, re-created, and/or newly created. We want the home page, cleanliness, and functionality to emulate as in __________'s home page. The functionality of applying should be available right there on the sight. The colors, banner, fonts and layout need to be cleaned up/re-designed possibly. We are open to use of little bit of flash, sound, etc. The site should come through as award winning with a "wow factor." Our webhost does provide for ASP and SQL Server support if required, again..please advice. I am laying out our work to be done described in 3 ways: 1. Applicaiton Flow 2. HSG - Description of Work 3. Recap - More Details of what we need done Application Flow 1) User visits the site, and click on "Apply Now" 2) No registration is required, everyone will be able to fill-in basic ASP Form(s) (Site does not have to be in asp, please propose best solution) including contacts, education background info., and employment history, etc. 3) Attach a resume at the same time 4) Submit the above data and attached resume to (1 or more) e-mail address include the following features: - when check box is checked, exact data will be copied from identical data-fields to another. - when a list of preloaded forms are required for download. Recap - More Details of What we need done We are looking to getting an online application uploaded/re-done and its functionality taken care of as quickly as possible for [login to view URL] . This way healthcare professionals can go to the site, apply and we get their applications routed to our email id's right away with the correct subject lines and in clear and well presented format. We would like the site, the forms and functionality to emulate sites like [login to view URL] . Please advise how best to proceed. We need forms designed: A button should be available "Upload Resume" so the applicant can fill out name, address, phone number and quickly attach/send their resume. Individual applicants should be easily able to apply through an application (it should say, the submitting the application is as good as signing on a written application). Application should arrive so we can print it and file it easily. Also should have functionality through "format" button so it extracts data from application and converts it into a format we can present to hospitals that is same everytime. We should have option to print in word or convert to pdf, etc. Form should be available so clients/hospitals can submit their job orders/requirements Data from both inidividual and client side should go conveniently into database with various fields--I prefer SQL Server and MS Access functionality- should provide easy merge or or transfer of these fields into a contacts base that works with Outlook or any other easily adaptable format. Logins (user id/pass) designed for: Employers/Hospitals/Facilities Employee login (all forms about benefits, accounting, time sheets, etc should be available here) At executive login level, we need to be able to shedule various nurses locally and across the country in many different hospitals and keep track of who is working which shifts and times as temporary, per diem, travel assignment, temporary to permanent, or permanent placement---basically need some online shceduling software integrated/uploaded (HEALTH SOURCE SCHEDULER) to work or something else you can suggest. Need help here...perhaps you research on net. We should be able to post online jobs on our site very conveniently (as Intelistaf, White Golve, Nightingale,etc) Online testing (where we can give many candidates different logins/passwords of our choice) to do online tests --need help in finding US nurse and other occupational easy to administer and upload tests Apart from this you will need to gather our thoughts, integrate, organize and come up with a systematic flowdown of items. Right now parts of the website are too much geared towards international and foreign nursing. We need to move all such content to a separate area within the sight which sends the message to local clientele, yes we also do this (foreign/international placement of healthcare professionals) but it is NOT what we primarily do. Here are most of the specs of what we need accomplished. We will need input to the changes we require on specific pages of our site. Here you will need to work with us and give us your professionally recommended updated text/content, ideas and changes. We can gather, organize and send over the basket of enhancements. Here are some thoughts but are not necessarily organized. We'll need your input to facilitate. We need our website enhanced/redone so it is very professional, fun, and clean. We need mostly web designing and some web development. Adobe photoshop, maybe Flash/fireworks, PHP, HTML, ASP.net, SQL Server, XP should be the way to go but do not have to be. All code separated and organized for us to later easily access files. The dates for exams are old and there maybe new NCLEX data/dates. Our site should allow scalability.
