Map labeled with 3 locations (2 hotels and one restaurant)

I need to get a .jpg made of a small map that I will get printed and the put into bags that wedding guests will recieve when they check into their hotels. The map should be 4" x 4" and in a Mapquest style, but surrounded with a clean dark orange border. The map itself should be grey.. The three addresses on the map should be marked with a star and then text description of what the place is next to the star. Here are the three things and addresses that need to be labeled: The Mayflower Hotel 1127 Connecticut Avenue Northwest Washington DC, DC 20036 (star and text should be dark purple) The Beacon Hotel 1615 Rhode Island Ave. NW Washington, DC 20036 (star and text should be dark purple) The Army and Navy Club 901 17th St. NW Washington, DC 20026 (star and text should be dark orange, same color as border of map) The bottom of the map (in the space between the border and edge) should say: Please note: Jacket REQUIRED for entry to The Army Navy Club
