Build a quiz html5 or php

Hi, I want a quiz for my kids school we doing, im doing this FREE for the school so please keep this in mind when saying a price. What i want it, Parents have to register name, email and child's name, then it sends them to the quiz page, it then says "You are about to start the quiz, you have 3 mins to complete this and only get 1 chance" The person presses go and its starts, It needs to get the questions and answers from a questions file and be a)b)c) answers and the last question a number value like "how many people live in London" All this information needs to be kept with that user registration like time took and answers so we can look back at the end. User / device / ip can only get on chance so needs to be logged. -------------------- A voting page, this page is a basic page, the kids come out with numbers on them. then at the end the parent goes to the vote page (active times by a config file like start=1530 end=2250) On this page is a display of numbers, the person clicks that number, it says are you sure you want to vote for number , yes ok . its then logged that number. Again only 1 vote per ip / device. All votes must be saved so can be displayed at the end. --------------------------------------- I hope that all makes sense.
