Create detailed form for website

I want to build a form for my website which gives website visitors the option to fill out details and various options and selections to produce a estimate for a new home build. Front page will have a few boxes, some required (Name, email etc) then a 'Start Estimate' button to start the estimate. There will be approx. 10 questions with 3-4 options per question that the user will select. [Example] 1. How many bedrooms and bathrooms do you require? 1. 2 bedrooms, 1 bathroom 2. 3 bedrooms, 1 bathroom etc etc These will be set up in a drop box neat style and each will have a preset value (ie. 2 bed, 1 bath = 150m2, 4 bed, 2 bath 300m2) Each m2 will have a base price of $1800.00 Other questions will include options for carports, garages, pool, landscaping etc with various options having various values. I want to use m2 as the main calculation. So the option of a 6 x 6 shed and a 12 x 6 shed, with a m2 price of $700, will add onto the overall price. At the end the user is presented with a estimate based on the selections. The most important thing is for it to be very accurate and very well presented. I need it to be very easy to upload onto a dedicated page on my website. I may need assistance with this. Please register your interest. Samples of work will be requested to prospective freelancer.
