I would like to have a pdf form sent back that can be edited by customers online ideally and at end they press submit and then form responses are emailed to client email. I have plugin formidable with all extensions including signature and user registration. It would be nice to have this setup, and then we can have a plugin to approve users for the role of Wholesale. All normal users of the shop page should still be able to register without approval.

Hi, I specialize in creating custom-made, interactive PDF forms, including all of the scripts required to make them work perfectly. I had a look at the Word file and it should not be a problem.
