Poll generator for my staff - Already designed

I need a small php/ajax/mysql solution where users (my coworkers) can suggest options for companytrips with differerent prechoosen fields like budget, location, accomadation etc. And when they submit they get sent to a page with a uniqe ID in the url where you can vote on the selected options and also share the page with other coworkers (vote.php&id=UNIQUEID). And only 1 vote for each field pr IP.

I have already designed the two pages where you can see the functionality static. You need to code them together:

1. Possibility to add options to the different fields. And they appear as in the design

2. Possibility to delete option you don’t want

3. Possibility to add Custom field

4. If they do not enter anything in one field. The field is not a part of the voting poll on the next page.

voted.php&id=xxxx

1. This is a page who is the unique ID who can be used to send to other coworkers

2. Should show all fields who are filled out. With the options chosen by user.

3. Possibility to vote 1 time for each field based on IP.

4. The options should who how many votes it got and also show this with a green visual graphic as displayed.

5. Show title of poll in the top as displayed
