Short Php form Choose text from Mysql and send email

Very Simple project to send a email. PHP / Mysql Need to create a form that will read different email bodies from a table in Mysql and place their title in a combo. The form will have one input field for the email TO address, and will show a standard email address (saved in a variable in the form) with the email FROM. Combo default option (message id) and user email may come as parameter when form is loaded. User chooses one message from the combo, clicks a button that says PREVIEW and a new screen shows the user a preview of the email (html email). 2 options available: Send / Cancel If send is choosen, email is sent and email to / body are saved in a history table. If Cancel, system goes back to first screen. Very easy, should take less than 2 hours of work for a pro like you. ONLY FOR EXPERIENCED USERS WITH POSITIVE FEEDBACK PLEASE.
