Webform Data to a secure MySQLi Database (Laravel 5.4)

Hi Freelancers, I am seeking a very simply webform that will store the data into a highly secure mysqli database. I would like for the script (the webform) to be created using Laravel 5.4 and I would like for it to be highly secure and free from any XSS, CSRF, SQL Injections, and/or Spam. I simply need Full Name, City, State, Zip Code, Email Address, a Captcha (to prevent bots from submitting, etc), and a Submit button. Once the "submit" button is pressed, I need it to echo a basic message such as "Thank you, you have submitted successfully." All fields are required, and if someone has already submitted it should echo something like "Your email address has already been used, thank you"
