I am seeking a freelancer with expertise in designing a visually appealing and well-organized worksheet. My project involves creating a sample worksheet that is not only functional but also aesthetically pleasing and easy to work with.

**Requirements:**
- I already have a mockup that includes all necessary elements such as Title, Heading, Columns, and Rows for data entry, as well as Instructions or Guidelines.
- What I need is a transformation of this mockup into a cleaner, more structured format.

**The Ideal Candidate Should Have:**
- Experience in graphic design, specifically in creating forms or worksheets.
- Proficiency in design software that can handle page layout and styling.
- A keen eye for detail to ensure clear and distinct sections are properly labeled and formatted.
- Ability to apply consistent styling and formatting to enhance readability and user experience.

**Project Goals:**
- Revamp the provided mockup to emphasize clear divisions between different sections of the worksheet.
- Implement proper labeling and formatting techniques to guide the user through the worksheet seamlessly.
- Apply a consistent style and formatting across the worksheet to maintain a cohesive look.

By fulfilling these requirements, the worksheet will not only be more visually appealing but also more functional and easier for users to interact with. Your creativity and expertise in layout and design are crucial to the success of this project.

Please remove the black sections and combine tables that look like they go together. Do not change numbering eventhough numbers seem like they are disconnected. Thanks.
