At [login to view URL] you can see our project. Another developer created this website so far but it is not finished yet. We are looking for a freelancer to finish this project within 2 or 3 weeks.
The site should represent a process of calculating a premium and apply for an insurance. Once a premium is calculated we need to receive a mail with the information of the applicant (which is filled in at the homepage). If the applicant continues to fill in the forms he ends up with having arranged an burial insurance. Then again we need to receive a mail. Also the client then needs te receive a confirming e-mail.
Homepage adjustment:
The applicant needs to have the option to choose for 1 or 2 adults to be insured: '1 verzekerde' or '2 verzekerden'. If choosen for 1 verzekerde then the premium is calculated for 1 person (based on age, duration an amount). If choosen for '2 verzekerden' the premium should be calculated for 2 persons. We have attached 2 jpg's to explain how this need to function.
Second page:
On the second page of the site you see a left and a right area. In the left area the premium need to be calculated based on 4 items which can be found in the attached Excel file named Left Premiums. The 4 items are: 1 or 2 'Verzekerden' (Dutch word for insured), age (date of birth), duration (standard: 30 years) and the insured amount (options you can see at second page: 1,500, 2,500, 5,000, 7,500 or 10,000 euro). The last 3 items can be found in the Excel Left Premiums. If choosen for 2 verzekerden the process of calculating needs to be done for 2 persons. This will end up in a premium for 2 persons to be shown at the second page. At the left side of the second page we show five premiums. An applicant must be able to choose on of these and continue the process of requesting the insurance.

The premiums should be placed in a database we can fill and change ourselves. The database consists 3 items: age, insured amount, duration of the insurance.
Next to that there are several items with the site which need to be done:
- The backgrounds should upload faster then they do now. We want to be able to change the backgrounds ourselve
- The site must be responsive and also work on a mobile phone. It's necessary that the process of calculating premiums and request the insurance always works properly
- At several places fields need to get mandatory
- At several places visual and textual adjustments need to be made
