(PHP) Modify code page of a form to upload MP3 audio

The attached php page allows a client to upload an image via a web form.
Convert this code for “upload audio files” in mp3 format, instead of “images”. And add the following restrictions:
1. Restriction # 1:
Maximum file size for each audio: 100MB
2. Restriction #2:
The client can only store 5 audios in total, in the database. If the client tries to upload more audios, then display the following message:
Unable to upload more audios. Maximum number of audios: 5

Is it only mp3 files that are going to be uploaded? Or are there other audio files with different extensions as well?
