PC Telephone - Voice Over IP

Hello All Coders,
I am looking for a Voice over IP solution as follows: Users can dial by entering the IP address of another party and that party is alerted by a ring which can be a tone or popup window, etc. they then use PC microphones to speak, headsets to listen, etc.
My goal is to allow Help Desk personnel to speak with associates wile on calls, without having to use the rotation phone line.
Here are some features and specs:
Needs to send and receive on our supported platforms (Win 98, 2000 and XP)
Dial and Ring features as described above
A simple Ping feature to make sure the receiver PC is online
An IP identifier (i.e./ Host IP [login to view URL]) for DHCP reasons
An address book feature to store IP addresses with a Name beside each (selectable IP addresses for auto-dial would be welcomed as well)
