Credit card Form to my shopping cart

Hi,

1- i need a credit card form designed and integrated to my website.

the design and the colors of the form will match with my website design.

the form will capture the credit card info and record it to admin.

it will be offline credit card processor.

2- in my shopping cart, when an order is placed , i need an email to come to my email address,about the summary of the order.

the bidder will be professional about e-commerce websites and online payment systems.

also good php knowledge.
