Build a single page websitee

I need a single page cash back page. There are essentially 3 things needed on the page

1) Vendor registration / login - a dedicated code will be created for a vendor once he registers with us. At any stage he can login and see who all have used his vendor code to get cashback

2) Activate cash back - A simple popup asking user to input a vendor code and other details. As a result a new customer specific code (which will work now become a vendor code ) will be generated and this code needs to be approved by the admin

3) Claim cash back - a simple pop up asking for details to be submitted

I have a basic design in mind which can be shared once a quote is finalized
