Form creation - Wordpress

I need 4 small pieces of work configured on a wordpress website.
1. A Newsletter subscribe form (MailChimp)
Users would need to fill in:
-Name
-Surname
-Type of business/work
-Location
2. New wordpress menu item called "Vacancies/Opportunities"
Under the above menu item you will have the following sub menus
- Looking to for a job
- Looking for Virtual Assistant services
- Join our associate, partner and collaborators network
Each of the above 3 sub menus will click through to a form ( see the attached excel doc that lists the questions for each of the 3 forms above) Its important to note that each of the forms must have a file upload option whereby users will upload their ID etc.
