Design and Print DVD Labels and Case Inserts

Need 25 dvd labels and 25 inserts designed and printed.
Provide examples of the DVD Labels--in the following formats--Modern script font, Black on silver or a light gray background. The label should contain the following: Part I - Memorial Service, Part II - Reception, Part III – Epilogue, Part IV – Remembrance. Also put the attached photo on the Label. Do Vertical and a horizontal layout. The title should be:

For the inserts:
Size: 4 7/8 in x 47/8 in
The inserts will be a four page foldout. Page 1 will be the attached photo, On the reverse (page 2) will be the next attachment (Need to remove "DRAFT" at the top of page)
I would like it to be centered on the back side of the photo cover.
The remaining pages, 3 and four are attached.
