Calculate production dates based on sales dates. 1 Entry form / 2 reports

This projected could be created in excel, access or another easy platform. At the end I always want to have the option to export to excel in case I need to manipulate datea a little further. My needs are:

1. Create a screen for user to enter info. All Dates must be in the format YYYY-WW (MM/DD). It needs to be a pull down calendar or something easy to use.

2. Create a propagation report based on what user entered in No.1.

3. Create a production report based on what user entered in No. 1
