iPhone App Simple 2 page for form submit

What does it do: First Page: A logo on top or background. It allow user to input few variable (6-10 input) such as name, age, etc. Then user can submit the form 2nd Page: It validate the data (such as email, phone number, need to connect to our http server to get the validation), if valid, show a message and privacy statement. User can either "Agree" or "Back" to change the data. If user click "Agree", the data will post to our http server. the apps only need to return some message (from our http server, such as successful or fail message) The apps is so simple, we only have 1 setting. which is language, user can choose either Chinese (default) or English This apps copyright and licensed is fully own by us. programmer cannot resell, reuse any code in this project without our authorization. If this application is succeed, we will keep improve it, so you can think it is a long terms partnership to start with. We can't disclose too many info, but the form input data is simple, just text filed, drop down menu and some combination of radio button. the no. of input field should be about 6-10, not more than 20 type. Payment: ============================= We escrow 50% upon the project start, we test and submit to app store for approval. once it is online for download, we complete the 50% and pay the rest. If any reason, this app can't be online. Programmer either change it until it can be online or just cancel this project and refund the money to us. as if the project can't online, it is meaningless for us. any question are welcome.
