Physical Therapy Office Management Software

Looking for a programmer to code software to manage a physical therapy office. The software will have the ability to allow editing and creating new patient files. Also, reports that can be sent electronically for billing purposes. An accounting back-end will be integrated to keep track of the charges for every visit. The software will be web-based with the ability to allow setting of permissions .... some experience in writing medical software will be helpful. ## Deliverables Good questions 1. We are tracking patients one to many relationship with regards to visits. We are tracking the time recording the time the patient is seen by the therapist and what service(s) were provided ducring that period of time. All this is tied to a back end part of the application that allows the adminsitrator to set, add and edit the service, cpt code, and amount charged for every X minutes (15 being the default) of therapy. We are also tracking the employees of providers in relation to their qualifications (PT, PT, SLP or any other), their license status and expiration dates etc. Again, the adminsitrator is able to add employess and assign permissions for say adding patient information, editing etc. The application would also require the ability to schedule providers in 15 minute blocks. Again the scheduling is managed only by the individual with permissions to do so. 2. Once a patient has been treated, a bill is printed and their co-payment recievied. However, the bill to the insurance company will need to be sent electronically. This is the part I need to do a little research on. I believe understand that there is a specific format in which to send the bills electronically and it would be a matter of interfacing with a clearinghouse or the insurance company directly using an API. I will do a little more research on this and get back to you. 3. The software will not be tied to a 3rd party software. Rather, the software will be able to run reports to display total revenue for the month, week, day; display the most profitable services. When payment is recieved, the payment will be entered in the software so that we can track outstanding amounts. A seperate software will be used to track assets, rent and the like. 4. In physical therapy, we typically see patients who have been refered to us by a physician or self refered. Either way, the patient has a medical diagnosis that the physician has determined- say raptured achilles tendon. When we evaluate the patient, the patient is given a physical therapy treatment diagnosis or diagnoses say "pain in the ankle joint", or "abnormality of gait". The patient can have more than one diagnosis - medical or treatment. When the patient comes in, they may have documents from their physician, insurance cards and the like; those will need to be scanned and saved to their profile and placed in once of many standard folders created by the adminsitrator. The initial evaluation allows the therapist to assess the patient and determine the plan of care. The plan of care will include the services that will be provided say therapetic exercise, therapeutic activities and neuromascular re-education. The plan of care will have short term goals and long term or terminal goals. Depending on the part or parts of the body that need to be treated, the therapist will perform specific tests to rule in or out certain pathologies. For instance a patient presenting with shoulder pain may recieve a test called the hawkins kennedy and another called the speeds test to determine how to treat the patient. Though the admin section, I need to be able to add tests to body regions with a description of the test, video file attachment and PDF documents. There are many tests and sometimes the therapist needs a refresher on how to perform the test. After the first visit, the patient can be seen by a physical therapist or a physical therapist assistant. At each visit, a progress note is written to document the progress of the patient in relation to the goals that were set initially. In fact, it would be good if the software would allow the therapist to see the original goals and note how the patient is doing with each goal so that the summary for that day will show comparative improvement, stagnation or decline over time. If the patient is seen by an assistant, the 5th visit always has to be performed by a therapist. At this visit, the therapist may add more goals, discharge certain goals or modify certain goals. Regardless, the therapist should explain why they are modifying, changing or discharging the goals. The software has to track the number of visits by an assistance and prompt that a therapist treats the patient for at least part of the time on their 5th visit. In other words, the scheculing part of the app need to allow treatment of one patient by 2 therapists. The scheduling should also allow booking more than one patient per time slot for a therapist. The therapist will ask to see the patient a certain number of times a week. Failure to show up without notifying the patient is called no call no show. After a predetermined number of no-call no shows, the patient will be discharged and may be liable to pay a fee. Again, the therapist has to document that the patient was no-call no show and the administrator should be notified through the software alert system so that they may send the patient an email, letter or call them. At the end of treatment, a therapist will discharge the patient based on meeting the goals that were initially. Discharge can also be because the patient has reached a plateau where further rehabilitation will not cause and further gains in physical ability. After every treatment, the patient is given a home exercise prescription. This is a list of exercises that they should perform at home. I would like to help the patients with compliance by sending email or text reminders once or twice a day. Also, to have then log to our website and review the exercises, goals and progress toward goals. In healthcare, once a document has been completed and signed, it is official. Any changes to the document have to be disclosed and signed off electronically. So, if in a progress note a therapist typed 20 degrees of flexion instead of 2 degrees. Correcting this would should a stike through the original and would have the date and the therapist who made the change to show that there was a change after the fact. Every note needs to be electronically signed by the therapist. Notes that created by student therapists or assistants need to be co-signed by the therapist. Again, an alert should prompt the therapist to co-sign any pending notes by students or assistants before sending the notes the charges to the insurance company. The admin would therefore be able to add therapists and assign assistants and student who work under them. Finally, I would like to remind the patient of their appointment and time 48 and then 24 hours in advance by email or text to ensure that the number of no call no shows is mininal. As far as ultimate platform. The software will run on an apache server with PHP and a MySQL front end. I have setup a development area on godaddy but till possibly be porting the applicating to run inhouse on a windows 2008 server with IIS with PHP support and MySQL server. The client will run on a desktop at the front office using IE8 or Safari or Chrome and Android Based tablets and regular laptops in the gym. Typing will be as minimal as possible so we will need to allow reuse of phrases and creating templates to allow point and click for most care issues like goals.
