Checkbox not updating & re-route Delete input to my FORM

I will send codes or file so you can CHECK & edit then send back, i upload for trials.. Fix 2 things. #1. Fix I want to use my submit input button or form to delete my image instead of when i checkbox then confirm popup then deletes (Everything works). you must keep calling this file though "[login to view URL]" see SHOT [login to view URL] & i want the confirm popup soon after i press save changes button. #2 Fix. When i checkbox enable checkbox is not UPDATE in DB, my UPDATE code is not there or is wrong..I did set values to 1 to check in site and does work and keeps checkedboxed..See SHOT If i don't like the code you do i will be straight forward with you.. Pages are in my Admin panel so i'll do the testing
