Feedback form

I have this page on my website <[login to view URL]> I have attempted a feedback form within my own designed fireworks / dreamweaver template. I have all the boxes and content in place however because I designed it outside of dreamweaver some on the line content for example: Panda Anti-virus Titanium Edition 2005 ..... has gone onto two lines and parts of the page look untidy. Also I have the Text "SUBMIT" towards the bottom but I am unable to code the selections so that it sends the selections and results to my email for someone to request a laptop quote. I don't want to build in any prices as these change frequently so I just need it to act as a simple feedback form to collect the persons name and e-mail address, laptop selections and post the results to my email. Then I can email them a quote back the next day. I would suggest using javascript as I am not totally sure if my hosting company uses php, asp or cgi. Finally, If this does not make total sense please feel free to question me about anything and I do understand that you may not be able to use the coding I have already done and will probably have to start again from scratch. Thanks Mat ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform To work within the template design I already use. Must work in latest internet explorer versions Javascript prefered, however if it can be done differently then I am happy with suggestions I would like to be able to open it in dreamweaver or if i can have coding to put into my website template within DW MX
