Copy and paste php code into a template

I have a site: loveactuality.com. I have the php code for that site that runs it. I need someone to take that code and put that into a new template. Make all the links to work. To sum it up. I will give you the code you need and also the template. Someone needs to make them married together by making sure all the link work with the old code inserted into the new template. Make all the functionality to work properly. Please see attached.
