Need HTML coding for my online website.

I need to have a "Cart Note" for customers checking out on my store. The problem is five countries don't display (American Sumoa, Federated States of Micronesia, Marshall Islands, Northern Mariana Islands, Palau, Puerto Rico) when checking out under the country list. They are considered US territories, so customers who order from these areas need to know that the must select United States as their country, and then their country will be selected under the state option.
