Sudbury Media Inc. Need immediate programmer who wants to work with me on this project. Will require full rights to code to do with as I wish, and also license from the programmer. We require full copyrights for this project. I need a one page website for Sudbury Media Inc. with an online contact form and logo. All contact information sent through the online form will be sent to an admin where we can manage the contacts. Sudbury Media will be an internet portal and also looks for entrepreneurs to represent their internet based tools for web business. We need a top notch professional look. Here are the only instructions I have for this web page. Other things will be added to the site at a later date. 1)Make a Logo for Sudbury Media Inc and also write it out in full “Sudbury Media Inc.” Try to get the message across the we are a Media company. 2)Somewhere on the top of the webpage have an Email Us button. 3)Our email for Sudbury Media will be provided. 4)Leave area for the text which will be about 10 to 20 lines in the middle of the page. Text will be provided when awarded the job. 5)I have provided an example online form at the bottom of this message. I want all the code to operate as in the example online form. 6)Text at the top of the online form should say Contact Form instead of Personal Information. 7)Please include on the online form the fields as shown below but with the following changes. Remove the field Company Name. Add a field for Email between the Last Name and Address 1. Add a field to say Best Time to Call at the bottom of the form under Fax Number with a drop down menu with the following choices. morning, late morning, afternoon, late afternoon and evenings. 8)Please change the Sign Up button on the bottom to Send Form Now. Make sure that all information from the online form goes to Sudbury Media Inc. email and admin. 9)The Admin will store all contact information. This information will be instantly forwarded to admin when a visitor enters his/ or her contact information on the online form. The Admin will have the ability to view, add, edit, or erase each contact in the data base. Admin will have the ability to keep a running note log for each particular contact. Admin should have the ability to forward all or part of the leads to any email address. Leads will be stored by the most recent date entered. Admin should have the ability to view the contacts in all different categories such as by name, country, area code, order of sign-up, partial searches, partial searches, etc… 10)The results submitted to Sudbury Media Inc. email should be listed in the following format: First Name, Last Name, E-Mail, Address, Country, State/Province, City, Postal Code, Phone Number, and Fax Number. 11)The attachment is the example of the online contact form.
