MySQL/PHP Help

I have a form I'm working on, where you can vote yes or no for different fandoms. It uses a database. I copied and pasted the data dump for the database (the info is for testing purposes). Inside the zip I've attached you can find the php file with the form and a screencap of the page of the form. Basically, what I want, is for all "fandom"'s that have the same "member" name to show up on the form, which as it stands they will, however I want to ALSO be able to click one of the radio buttons (yes = 1 or no = 0) and then in the UPDATE query, I want it to change EACH data row where either the ID is the same OR where the fandom = fandom and the member = name are the same (I'd think doing it by ID would be easiest but I am not sure).
