139384 PHP MySQL Site

Hello, we need a basic php site. There doesn't have to be any real style to it other than centered on the page and look like the attached pictures. We run a weekly poker league and need three seperate log ins. One for the players, one for our clients, and one for the admins to have control over all of the site. We already have a website but I can't seem to get a hold of the programmer that designed it for us. I think he has finals for school but we need this done asap.
