Code for opt-in form

I am looking to get some code written for a unique style of opt-in form I saw on a website. Not sure what kind of code this is using. The form can be seen at [login to view URL] . If you scroll down a bit you'll see the opt-in form. When you enter your name and email, it removes the opt-in fields and displays a message stating that the form has been received, etc. The main benefit is that it keeps you in the same spot on the page and doesn't refresh the page or move you to another page.
