Outlook Add in icon switch

We need an example application compiled to run as an Outlook add in that can change text and graphics of a Toolbar button. This application should show up on the toolbar as a button in Outlook. The button should display a small icon and the text "Sample App". When the button is selected it should present a menu with four items. The item text should be "Item 1, Item 2, item 3, Item 4". Each item should have a seperate graphic that is displayed next to the item. When one of the items is selected, the text on the button should change to "Item 'X'" where X is the item number and the graphic associated with the item should be displayed on the button. This project should be written in Visual Basic 6.
