PHP programming to add complex form to existing site

Requirements: php, JavaScript, mySQL database Description: You will need to add a complex form to an existing php website. The form will have about 110 fields. You will need to create the database table to hold the fields (field list and data types will be provided) You will need create the html form. Form will have some of the information pre-filled from an existing record. The form will be called with a parameter (jobId). Based on this parameter some fields will be filled in with the data from another table. The rest of the form will be filled in by user. Form will have a couple of areas where some of the fields are hidden. If user clicks on "Add more fields" these hidden fields will show. Use JavaScript to do this. All date fields need to use a calendar popup date selector. (we have existing function for this that you can use) Some of the fields' values are calculated based on user input. After all the fields are filled in, record data into database. You will need to create an edit version of the form, where users can edit previously submitted data. (This website has many other forms, so you will be able to reuse some code or elements such as the calendar popup.) IMPORTANT: Your code must be clean, simple and easy to understand.
